Consistently? Can reproduce on test? sometimes mobs manage to get into locked or protected areas, it even happens at my main base in north haven... like if they spawn on the line between unprotected and protected... that sort of thing. The density of spawn in nether may or may not have been a factor here. Opinions?
A missing ladder may give that impression, but I very much doubt anything would spawn inside ladders.
In any case, if we know the position we can work out what happened.
