No idea at the moment how to do that. Would have to take a look at the itemframe mod. And even then - it may be available for entities but not for blocks. So I'd still need to get a model for a 3d wheel somehow.
the itemframe does it by using a wielditem entity, which uses the 2d texture, extruded. there's no immediate way to use that as the mesh/nodebox for a node. someone could probably just define a nodebox by hand. not hard, but tedious.
There are some open source voxel editors, but you would still need blender to properly scale/position the model to be exported to minetest...
Also, should be not too hard to just use blender too - enable snap to grid and move/scale boxes around... But that requires a bit more knowledge to make it work I guess
Well, I did it the blender way...
I'm not the biggest fan of this being the biggest model in cottages (I mean file size). Hmmm.
Also, I can try to make a tutorial that works for this "extruded" look...
I know Alias demands tutorials, I just wonder if anyone would actually use them to make stuff XD
If it's a model anyway - perhaps it can be even more round? Some things are round already and seem to be fine regarding size - i.e. the handmill.
Tutorials are fine. Not all are as reluctant to use Blender as I am :-)
I was thinking opposite - that the barrel should be made from voxels :p
But okay... what's the good shape for a wheel? Octagon?
Tutorial video exists, don't know what next... I hoped Soko would try at least X)
I like it this way it is now.
@Sokomine should I clean up and PR the voxely version?
If you need universal model which works with any texture of size 16x16 8x8 4x4 2x2 (1x1 :P), feel free to use model in attachment.
Heh, that's clever :p
And this is also how default extruded items work, right?
Wait, this texture is 17x17, so it looks like this:
or something else is also going on?
Oh right. It doesn't work with this size.
If anyone ever wants to cherry-pick this:

This commit has 24 invisible quads removed, so prefer this one to that zip.
I'm not sure where to PR this? to githut? to your-land? meh
ideally to https://github.com/Sokomine/cottages, but i've got some moribund PRs there currently, and i don't know how to get them moving.
gotta get the soko moving above all else
Maybe I should try to make a rounder wheel, before I forgot how to use blender again...
Soko: you prefer this one?
Cleaned up round model file:
wheel-round.zip
and voxely one (should be same as in that repo, but still)
