Hmm, this could also be lag, an unfortunate delay or an engine problem. As we have found out in a past issue, the 1/16 slab is in your hand, but is not shown because it is so flat, barely outside of your screen. Could MT interpret this as the slab not being there as a light source, although it IS in the hand?
no, that's not how this feature works at all - there's a check for whether the thing the player is wielding is defined as a light source in its definition, and then, because objects cannot emit light, it will make an invisible light source where the player is standing (or directly adjancet to that node), if it is air. my guess is the player was standing in a bunch of ladders/grass/whatever, and the mod didn't find a safe spot to put the light source.
This is not a bug, but it is confusing to players, so perhaps some sort of documentation is needed.
